Preparing for your visit
- Log in to your patient portal account. Don't have an account? Click here to create a portal account.
- Write down any questions you would like to ask.
- Bring your ID to your first visit and your insurance card to each visit.
- Bring a list of all medications you are currently taking – dosage, how often and what time you take them. Be sure to include supplements and over-the-counter medications.
- New Patients - You may choose to fill out your New Patient Information Forms and bring them with you - however, the majority of the information on the paperwork is already in our system so you simply verify everything when you arrive.
- If you have imaging on a disc/CD, that was ordered by another doctor's office (CT scan or MRI), please upload it here or bring it with you.
- Arrive 15 minutes before your appointment time so we can get you checked in.

Billing and Insurance
- We are committed to understanding your benefits and providing you with a cost estimate for your care before your appointment
- Before your appointment, please inform us of any changes to your information such as name, address, phone numbers and/or insurance information
- We will collect for today’s care and any outstanding balance when you check in
- Financing is available through Care Credit
- We do not accept Letters of Protection (LOP), new workers’ compensation cases, Medicaid insurance, and we do not file auto insurance claims (also known as 3rd party insurance)
- If you miss an appointment, or if you cancel or reschedule an appointment within 1 business day, we may charge a late cancellation fee of $25 for an office visit appointment and $75 for all other appointments.
- Neuropsychology Testing Appointments require a $100 deposit 3 days before the appointment. This deposit is converted to a no show fee if the appointment is missed or applied to out of pocket expenses if the appointment is kept. If a refund is due, it will be processed the day of the appointment.

- Records Requests FROM Kane Hall Barry - we have partnered with MediCopy services for all medical records requests for records from Kane Hall Barry to be sent elsewhere. Please call Medicopy at 866-587-6274 or go to their website at www.medicopy.net to request records. Records requests take 7-10 business days to process and may incur a fee per our financial policy.

Testing Information
EMG/NCV - Electromyogram and Nerve Conduction Study
EMG/NCV testing is used to evaluate the function of the nerves in the body, the electrical activity of the muscles, and how well and how fast the nerves can send the electrical signals. Testing takes approximately 1 hour and is conducted on your arms, legs or both. Please be sure you do not have any lotions or skin moisturizers on the area(s) to be tested. You may take medications as prescribed and eat/drink like normal. Please make prior arrangements for childcare as we don't recommend children be in the testing room for their safety as well as yours. EEG - Electroencephalogram
EEG tests are used to detect the electrical activity in the brain and can be awake (most common) or sleep deprived. Testing takes approximately 1 hour and is conducted by placing electrodes on your scalp. Be sure to arrive with clean, dry hair – no hair products including hairspray and gels and remove any braids and/or extensions. You may take medications as prescribed and eat/drink like normal, however, please AVOID CAFFEINE . VER/BAER - Evoked Potential Testing
Evoked potential tests can be VER-Visual Evoked Response or BAER-Brainstem Auditory Evoked Potentials. Testing is performed by applying small sticky pads/electrodes to your scalp and then tracking and recording the brain wave patterns in response to various stimuli. Each test takes approximately 30-45 minutes. Please arrive with clean, dry hair – no hair products including hairspray and gels. In addition, please remove any braids and/or extensions. Carotid Ultrasound/Transcranial Doppler (CUS/TCD) - Vascular Studies
Carotid ultrasound tests use sound waves to produce pictures of the arteries in the neck which carry blood from the heart to the brain. Transcranial doppler tests measure how quickly the blood flows through the blood vessels. Carotid ultrasound testing takes 30 minutes and carotid ultrasound with transcranial doppler testing takes an hour.
