Preparing for your visit
- If you provided an email when you scheduled your appointment, you can log in to your portal account using the link provided.
- Write down any questions you would like to ask and bring them with you to your appointment.
- Bring your ID to your first visit and your insurance card to every visit.
- Please be ready to pay for today's care and any outstanding balance at check in. Please note: We do not accept cash or checks.
- Bring a list of all medications you are currently taking – dosage, how often, and what time you take them. Be sure to include supplements and over-the-counter medications.
- New Patients - There are a few forms for you to complete before your appointment. The preferred method of completion is for you to use our self check-in link that will be sent via email or text before your appointment.
- You may also choose to complete your forms online by completing the Online New Patient Information Forms OR the Printable New Patient Information Forms below prior to your appointment.
- Please arrive fifteen minutes before your appointment time so we can get you checked in.
- If you are running late, please call us.
Billing
- We are committed to understanding your insurance benefits and providing you with a cost estimate for your care before your appointment.
- Before your appointment, please inform us of any changes to your information such as name, address, phone numbers and/or insurance information.
- We will collect payment for your care and any outstanding balance upon check-in for your appointment. Please note: We do not accept cash or checks.
- Financing is available through Care Credit.
- We do not accept Letters of Protection (LOP's), new workers’ compensation cases, or Medicaid insurance. We do not file auto insurance claims (also known as 3rd party insurance).
- If you miss an appointment without notifying us, you may be charged a no-show fee.
- Please review our full financial policy for any additional questions.
