Preparing for your visit

  • Bring your ID and your insurance card to every visit. All of our clinics require that we review your ID and insurance card at each visit. We keep this policy in place to combat fraud and appreciate your cooperation.
  • New Patients - There are a few forms for you to complete before your appointment. The preferred method of completion is for you to use our self check-in link that will be sent via email or text before your appointment.
  • You may also choose to complete your forms online by completing the Printable New Patient Information Forms below prior to your appointment.
  • If you provided an email when you scheduled your appointment, you can log in to your portal account using the link provided.
  • Please be ready to pay for today's care and any outstanding balance at check in. Please note: We do not accept cash or checks.
  • Bring a list of all medications you are currently taking – dosage, how often, and what time you take them. Be sure to include supplements and over-the-counter medications.
  • Please arrive fifteen minutes before your appointment time so we can get you checked in.
  • If you are running late, please call us and press 7. Follow the directions on the prompt and leave a voicemail to alert our staff.
  • Write down any questions you would like to ask and bring them with you to your appointment.
